It shouldn't come loose because the nibs are the same size and shape. Either the nib isn't in right or something is broken.

Yes, over time, cartridges can dry up. I had some old Sheaffer and Waterman carts that were half full.

Hehe. In my case, it might be habit. I also remember things better when I write them down, even if I don't ever refer to my notes. Like I don't forget appointments that I list in my appointment book.
So, thus far, the list of scary staining inks is: 1) BSB, 2) Kung Te-Cheung, 3) Aurora Blue.

That's really weird. I use BSB in a black Pilot 78G. If I hold it at a certain angle I can see how the ink has stained the feed & section, but nothing else has happened to the pen.
