    When I tried it just now, I found myself twisting pen body and cap away from each other to start, and continuing by just turning the cap.

    Since I never really thought about it before, I have no idea if I've been doing it that way consistently, or if the asking of the question has made me do it differently than would otherwise have been the case.

    I always turn the cap (with the left hand) and hold the pen with the right (no turning).
