That is a franken cap, i.e. a cap with a replacement clip.
The bump on the clip was put on pens like the Sheaffer Sentinel Deluxe Stratowriter, which used the bump instead of the dot. In that era the white dots only went on the Triumph nib pens.

The bump on the clip is for the ballpoint. Apparently some of the ballpoint caps did have white dots on the cap with the bump - I've owned three of them over the years (still have one). These were Touchdown era pens.
